Business Number: A nine-digit account number that identifies your business to federal, provincial, and municipal governments.
WSIB Firm Number: Firm number issued by the Workplace Safety and Insurance Board.
WSIB Account Number: Account number issued by the Workplace Safety and Insurance Board.
WSIB Rate Group: A three-digit number that the Workplace Safety and Insurance Board uses to set premium rates.
Ownership Type: The legal structure of the constructor's business. For example: a business owned by one person is a Sole Proprietorship. If you are not a business select Person.
Constructor Legal Name: The legal name of the person or business that is undertaking the project for the owner. The constructor may also be the owner if the owner contracts directly with the employers who work on the project.
Constructor Operating Name: The name by which people know your business. Operating name is sometimes known as a Business Name or Trade Name. If you are not a business leave this field blank.
Company Official Name: First and last name of the company official.
Company Official Position or Title: Job title of the company official. For example: Director or Owner.
Constructor Head Office Address: The location where most, if not all, of the important functions of the business are coordinated.
Telephone Number: Telephone number of the constructor.
Fax Number: Fax number of the constructor.
